This program is open to qualified adult students. New applicants will need the following items to complete this application. Cost/Fee information for these items can be found on the Driver's License Division's (DLD) website. Contact Tricia Cheney (801-593-2008) with any questions.
- Valid Driver's License
- Valid Class A Learner's Permit
- Schedule a CDL appointment and fill out the application prior to going to the DLD
- Valid DOT Physical Exam Card
- Find a provider on the registry. You must have the exam prior to going to the DLD & must take the card with you to the DLD
- CDL MVR Report
NOTE: If you are NOT sponsored by an employer that already has a regulated program then you will also need to submit the following items. Service costs will vary by service provider:
