With the full approval of the Pfizer BioNTech COVID-19 vaccine, and with new guidance from the Utah Board of Higher Education, Davis Technical College will require students attending class on our campus to have received the COVID-19 vaccine by December 22, 2021, with exemptions for medical, religious, and personal reasons permitted.
Students will need to complete a COVID-19 Vaccination Record form with your vaccine(s) date and upload a photo of your vaccine card.
To request an exemption, on the same form, please select Vaccine Type, then Exemption and choose the type of exemption you are requesting.
Failure to submit the vaccination record by the date noted above, may result in students being withdrawn from their program.
