Apply for Graduation
All students requesting graduation must submit the application for graduation (below). There are actually multiple forms available to you, depending on your circumstances. If you have any questions about which option you need to select, please check with your instructors or an advisor in Student Services.
NOTE: Students or guests requiring special accommodations during the graduation ceremony may contact our Disability Services Office (before graduation day) at ADACoordinator@davistech.edu or 801-593-2396.
